Caser Residencial is looking to add a Care Manager to its Caser Cuidados team for a residence in Lorca.

-Carry out the center's annual care planning.
-Determine the care planning for each resident together with the team.
-Propose the materials and tools necessary for the performance of their activity.
-Plan the management of the HR staff under their charge together with the center's management.
-Manage the cases entering the center with RATC and Management.
-Participate in the definition of functions and responsibilities of the team.
-Administer the resources of their area and provide guidelines to the team.
-Actively participate with Management in the definition of tasks and workflows.
-Coordinate the call, follow-up, and communication of PAI meetings, ensuring communication to the family and resident.
-Coordinate EM meetings and meetings with Gerocultoras, ensuring communication between departments.
-Organize and ensure the general training of the EM and the auxiliary team.
-Support the residence Management in the resolution of conflicts and difficulties that arise in the management of the residence related to their area.
-Resolve, or escalate to Management, any incidents that arise related to the service.
-Assume responsibility for the center during on-call shifts.
-Perform tasks specific to their professional category if necessary.

-Gestionar los casos que ingresen en el centro con RATC y la Dirección
-Participar en la definición de funciones y responsabilidades de su equipo.
-Administrar los recursos de su área, y da pautas a su equipo
-Participar activamente con la Dirección en definición de tareas y rutas de trabajo
-Coordinar la convocatoria, seguimiento y comunicación de reunión de PAI asegurando la comunicación a la familia y residente.
-Coordina las reuniones de EM y reuniones con Gerocultoras segurando la comunicación entre departamentos.
-Organizar y asegura la formación en general del EM y equipo de auxiliares.
-Apoyar a la Dirección de la residencia, en la resolución de conflictos y dificultades que surjan, en la gestión de la residencia relacionados con su área.
-Resolver, o traslada a Dirección aquellas incidencias que se susciten relacionadas con el servicio.
-Asumir la responsabilidad del centro durante sus guardias.
-Realizar tareas propias de su categoría profesional en caso de que sea necesario.
