About the role

The Fundació Germà Tomàs Canet of the Hospital Order of St. John of God is a non-profit entity whose mission is to support people in situations of vulnerability and risk of social exclusion through the management of programs and services to improve their quality of life and social integration.

Currently, it is looking for a Social Integration Specialist for the Llar Ronda Facility (Barcelona). What you'll do

Under the supervision of the Coordinator, the responsibilities are:

Responsibilities related to the person assisted

  • Monitor the protected individuals in the homes of the Autonomous Living Support Program.

  • Prepare and participate in support activities aimed at the protected individuals of the service and promote their participation.

  • Directly assist the people living in the facility and provide the necessary supports to improve their quality of life.

  • Manage conflict situations within the home.

  • Monitor the execution of Basic Daily Living Activities of the protected individuals.

  • Detect needs in the various aspects of the lives of the residents, whether personal or environmental.

  • Monitor and detect, when necessary, the correct administration of medication.

  • Provide emotional support and guidance to the resident to secure the human, personal, affective, and relational supports they need.

  • Coordinate with the reference team regarding visits to medical services, legal proceedings, or various procedures.

  • Accompany, if necessary, to hospital emergencies, assessing the situation and determining how to act during working hours.

  • Provide support in the purchase of personal items, food, clothing, and other items for the support and guidance of the person assisted.

  • Manage and justify the expenses incurred in their work.

  • Jointly prepare the calendar of actions and visits of the reference team.

  • Promote the dynamization, coordination, contact, and management of the various natural supports, community environment, and specialized services in the various areas (health, social services, etc.)
