About the role

The Germà Tomàs Canet Foundation, of the Hospitaller Order of Saint John of God, is a non-profit organization whose mission is to accompany people in situations of vulnerability and at risk of social exclusion through the management of programs and services to improve their quality of life and social integration.

We are currently looking for two Social Workers for the Refugee Care Program. The mission of this position is to support people in vulnerable situations, especially applicants or beneficiaries of international protection, with the aim of facilitating their social inclusion, personal autonomy, and access to resources and rights, promoting integration pathways and contributing to the improvement of their living conditions, in accordance with the values of the Hospitaller Order of Saint John of God.

What you'll do

Reporting to the Coordination, their responsibilities are:

General duties

  • Provide social support to the people served in their integration process.

  • Conduct intake and follow-up interviews.

  • Prepare the social diagnosis and identify needs and situations of vulnerability.

  • Monitor individualized work plans.

  • Prepare social reports.

  • Coordinate with social services, local organizations, and other professionals.

  • Participate in the facilitation of spaces and ensure compliance with coexistence regulations.

  • Support interns, if applicable.

  • Coordinate and monitor volunteers in collaboration with the person in charge.

  • Record information in computer applications and ensure their proper use.

  • Support the work team, if applicable.

  • Participate in training activities and professional development spaces.

  • Ensure the good condition of physical spaces and equipment.

Specific duties

1. Reception and access to the program

  • Assess referrals and manage actions prior to admission.

  • Provide information about the international protection system.

  • Conduct intake interviews in coordination with the social educator.

  • Prepare documentation and social reports for access to the program.

2. Social intervention and individualized follow-up

  • Develop, monitor and update individualized intervention plans (IPI) in coordination with the social educator.

  • Identify situations of vulnerability and refer to specialized resources.

  • Process financial aid applications.

3. Comprehensive support

  • Manage, process resources and follow up on the needs of service users regarding: o housing o coverage of basic needs o health o education o social network.

4. Resource management and coexistence

  • Ensure the proper functioning of the resource in terms of reception, coexistence, and use of spaces.

  • Ensure compliance with internal regulations and program protocols.

  • Participate in the promotion of shared spaces.

  • Promote autonomy.

5. Coordination and networking

  • Coordinate with social services, organizations, and public administrations.

  • Participate in team meetings and technical coordination spaces.

  • Collaborate with the interdisciplinary team in case follow-up, especially with the referring social educator.

6. Administrative and technical management

  • Record and update information in the program's management tools.

  • Manage and follow up on case files in the SIRIA application.

  • Prepare social reports and technical documentation.

  • Draft program justification reports.

7. Community action

  • Support the development of group activities and community actions.

8. Other

  • Carry out any tasks necessary for the proper functioning of the service.
