About the role

The Germà Tomàs Canet Foundation, of the Hospitaller Order of Saint John of God, is a non-profit organization whose mission is to accompany people in situations of vulnerability and at risk of social exclusion through the management of programs and services aimed at improving quality of life and social integration.

Currently, we are looking for Support Measures Referents at our offices in Barcelona, Sant Boi de Llobregat, and Lleida. The mission of this position is to ensure proper support and assistance for individuals served by the support measures program, providing ongoing case monitoring to improve the capabilities of those served, as well as raising awareness and informing the community about the service's resources.

What you'll do

Reporting to the office Coordination, the responsibilities are:

  • Carry out the initial and ongoing assessment of the person.

  • Provide initial reception to the person being served, inform them, and guide them regarding the Support Measures Service.

  • Identify personal needs in all areas and seek strategies and alternatives to maintain and improve the quality of life of the person served.

  • Foster the bond between the person served and the support measures assistant.

  • Develop, update, and monitor the Individual Care Plan, together with the interdisciplinary team, assessing the needs, preferences, and choices of the person being served.

  • Provide support and accompaniment to the person served through different grieving processes (death, loss of abilities, housing, etc.).

  • Establish, together with the economic area, the financial budget and prioritize expenses, as well as their monitoring and control.

  • Prepare the relevant reports.

  • Make periodic visits to the person served.

  • Assess authorizations for the different activities carried out by the people served.

  • Accompany the person served in situations that require it (activities, medical visits, transfers between centers, etc.).

  • Conduct the initial social assessment for the grant modules and provide the information needed to update them.

  • Work with the goal of building connections between the person being served and their referent.

  • Inform the family about the referent's and the organization's roles.
