Caser Servicios wants to incorporate a social worker into our team for our Parque Llevant Residence center.

Reporting directly to management. Your main mission will be to attend to the demands of patients and families who may need social assistance and/or resources.

Their main responsibilities include:

• • Inform, assess, and guide people who visit the Residence requesting a placement for any of the offered stay modalities.
  • Manage admissions to the center, including the collection of necessary documentation and the completion of contracts.
  • Carry out the welcome of residents and families, together with the coordination of the geriatric care team.
  • Participate with the rest of the multidisciplinary team in the preparation of the residents' Individualized Care Plan (P.A.I.), including semi-annual reviews of the same.
  • Monitor the assistance of the nursing service, in coordination with the Nursing Directorate.
  • Perform and update the social assessments of the residents.
  • Organize the distribution of residents' rooms and their seating in the dining room, together with the coordination of the geriatric care team.
  • Manage the needs and demands of residents' families, including receiving complaints, claims, or suggestions and conveying these to the Management.
  • Manage various administrative tasks for all those residents who require it and/or who are in exceptional situations such as the absence of reference family members and/or legal guardians.
  • Promote the integration and participation of residents in the center and, at the same time, prevent distancing from the family. Collaborate with the rest of the multidisciplinary team so that users participate in the residential services.
  • Manage relations with Social Services, health services, public administration, and public hospitals for matters related to residents and/or possible admissions.
