We stand out for two important lines of activity: home care for patients with complex chronic pathologies and medical emergencies in the home.
To achieve this, we have a team of proactive and dynamic healthcare professionals who are committed to preserving the lives of our patients.

Perform functions specific to their profession in the home care of patients under the Basque Country emergency contract.

Anamnesis: The Nurse collects data from the patient(s) and/or caregivers regarding the signs and symptoms experienced by their illness, clinical history, analysis of available reports, allergies, etc.

Physical examination: Physical examination, nutritional status, and psychological status of the patient will be performed.

Nursing assessment: With all the data obtained from both the anamnesis and the physical examination, the Nurse performs the assessment of the patient.

Performance of nursing procedures: The necessary nursing procedures will be performed, such as wound care, sutures, catheterization, etc.

In the event that a specific care plan is required until the assistance of another healthcare resource in the system is available, if the Nurse deems it appropriate, they will define a care plan where it will be explained in detail to the patient and/or caregivers and will be recorded in the clinical history report. Recording of the case in the clinical history.

Discharge from home/Discharge with referral: Upon completion of the intervention, the Nurse will conclude the intervention at the home itself; if deemed necessary, they will refer the patient, in accordance with the emergency coordination center, to the hospital. Inform their coordinators or higher-ranking supervisor of any relevant incident detected during the course of the service.
