We are looking for a Home Care Services (SAD) Coordinator! We are looking for dynamic and detail-oriented people, with the ability to communicate effectively with the families who have trusted us and define the caregiver profile that best suits their needs. With a client portfolio to build loyalty through the monitoring and advice of the contracted care services.
Perform telephone socio-sanitary assessments to identify the care needs of older adults and ensure appropriate and personalized care.
Define the required profiles for each type of care need, guaranteeing an optimal selection of caregivers.
Resolve doubts and provide information to families and users via telephone and email, maintaining effective and empathetic communication.
Perform continuous monitoring of clients, acting as their point of reference and ensuring comprehensive care adapted to their needs.
Act as a link between the client and the different departments (labor, administration, Dependency Law, etc.), facilitating communication and the flow of information to ensure coherent and efficient care.
Offer guidance on caregiver services, manage conflict resolution, and ensure client satisfaction through proactive detection of needs.
Schedule: from 09:00 to 14:00 and from 15:00 to 18:00, Fridays from 08:00 to 15:00
Temporary contract for substitution.
