Clínica López Ibor is composed of a psychiatry and psychology clinic, and a psychiatric hospital in Madrid, which bring together a team of more than 100 highly trained professionals, including psychiatrists, psychologists, therapists, and specialized nurses, who work in close collaboration to provide comprehensive and personalized care.
What you'll do
Currently, we are looking for a SOCIAL WORKER for the Clinic who will be responsible for the social assessment and intervention of patients and families in the field of mental health and psychogeriatrics. You will coordinate appointments, admissions, and discharges, collaborate closely with the multidisciplinary team in the definition and monitoring of care plans, and facilitate access to social, health, and community resources. You will drive coordination with external organizations and institutions, guarantee compliance with current regulations, and ensure the proper management of files. You will contribute to the continuous improvement of care quality and the experience of patients and families, promoting comprehensive person-centered care.
How you'll work
- The main workplace will be at Clínica López Ibor with occasional support at Puerta del Hierro.
