Reporting directly to the program coordinators, your main functions will be the following:
Assessment and reception of people accessing the service, in the social area
Design of the intervention line for the Personal Development and Socio-relational Area, agreeing on objectives and commitments with each individual.
Organization of appointments at social services and specialized centers.
Coordination with the general and specialized social services network, to favor access for the people served to the public system.
Collaborate with the social and healthcare team on general intervention lines
Actively cooperate in the development of assigned tasks and in the design of new proposals. Carry out the necessary tasks for the execution of the activities that make up the assigned Social and socio-sanitary area.
