We stand out for two important lines of activity: home care for patients with complex chronic pathologies and medical emergencies in the home.
To achieve this, we have a team of proactive and dynamic healthcare professionals who are committed to preserving the lives of our patients.

Perform the duties inherent to the profession in home care for patients covered by the Basque Country Emergency Contract. Collaborate in complying with quality, environment, and safety procedures across the different business units.

Medical history: The physician collects data from patients and/or caregivers regarding the signs and symptoms of their illness, clinical history, analysis of available reports, allergies, etc.

Physical examination: A physical examination will be performed, along with assessment of the latest diagnostic tests available in the home, the patient's nutritional status, and psychological state.

Nursing assessment: With all the data obtained from both the medical history and the physical examination, the physician performs a comprehensive assessment of the patient.

Medical diagnosis: If the information obtained is conclusive, the physician issues a diagnosis and codes it according to the ICD-10 code list defined by the client. If additional tests not available in the home are required, the physician will request referral of the patient to confirm the possible diagnosis.

Treatment and/or care plan: The treatment guidelines and/or care plan will be explained in detail to patients and/or caregivers and will be recorded in the medical record report.

Discharge from home/Discharge with referral: Upon completion of the medical intervention, the physician will discharge the patient from home care; if deemed necessary, the patient will be referred, in accordance with the emergency coordination center, to the referral hospital.

Report any significant incident detected during the provision of the service to their coordinators or immediate supervisor.

Degree in Medicine and Surgery, or an equivalent foreign degree, or at least six months of professional experience.

Realizar funciones propias de su profesión en la atención domiciliaria de los/las pacientes atendidos en el Contrato de Urgencias del País Vasco. Colaborar en el cumplimiento de los procedimientos de calidad, ambiente y seguridad de las distintas unidades de negocio.

Anamnesis: El/la médico/a recaba los datos de pacientes y/o cuidadores(as) relativos a los signos y síntomas que experimenta su enfermedad, antecedentes clínicos, análisis de informes disponibles, alergias, etc.

Exploración física: Se realizará exploración física, valoración últimas pruebas diagnósticas de que se disponga en el domicilio, estado nutricional y estado psicológico del paciente.

Valoración de enfermería: Con todos los datos obtenidos tanto en la anamnesis como en la exploración clínica el/la médico/a realiza la valoración global del paciente.

Diagnóstico Médico: Si la información obtenida es concluyente el/la médico/a emite su diagnóstico y lo codifica según el listado de códigos CIE10 definidos por el/la cliente. En caso de precisar pruebas complementarias no disponibles en el domicilio, solicitará la derivación del paciente para confirmar el posible diagnóstico.

Tratamiento y/ plan de cuidados: La pauta de tratamiento y/o plan de cuidados se explicará detalladamente pacientes y/o cuidadores(as) y se plasmará en el informe de la historia clínica.

Alta en domicilio/Alta con derivación: Al finalizar la actuación médica el facultativo dará el alta en el propio domicilio en el caso de considerarlo necesario lo derivará, de acuerdo con el centro coordinador de emergencias, al hospital de referencia.

Informar de cualquier incidencia relevante detectada en el desarrollo del servicio, a sus coordinadores(as) o superior jerárquico.

Grado en Medicina y Cirugía, o titulo extranjero homologado o experiencia profesional de al menos seis meses.
