We are looking for a Personal Counselor to join our Prevention team.
What would your role be? 📋
You will be part of an interdisciplinary team in the social field (psychology, social work, occupational therapy, among others), accompanying people over 60 years of age in the prevention of neurodegenerative diseases through the promotion of healthy lifestyle habits.
Your mission will be to empower and guide users in improving their health and quality of life, combining individualized intervention with the use of digital tools and interactive technology.
During the first few months, you will specialize in the initial assessment of health and cognitive status to determine eligibility for prevention programs.
What you'll do
Conducting initial health and cognitive status assessments using specific questionnaires and tests.
Design and monitoring of personalized Prevention Plans.
Continuous accompaniment of the person in their habit-change process, acting as a point of reference.
Comprehensive care for the needs of the user and their family environment.
Coordination and management of services associated with prevention and care (including specialized advice).
Detection and management of situations requiring additional intervention.
